Welcome to Elevated Veterinary Solutions LLC (“Company,” “we,” “us,” or “our”). By accessing our website and using our booking or intake forms, you agree to be bound by the following Terms and Conditions ("Terms").

1. Services & Professional Scope

Elevated Veterinary Solutions LLC provides independent relief veterinary technician services. By submitting an intake form, you are requesting a booking or information regarding these services. Submission of a form does not constitute a binding contract for a specific shift until a separate Service Agreement is signed and confirmed by both parties.

2. Information We Collect

When you use our intake form, we collect the following professional information:

  • Hospital/Clinic Name, Email, Phone Number, and Physical Address.

  • Practice Manager or Authorized Point of Contact Name.

  • Type of Practice (General, Specialty, Emergency, etc.).

  • Specific Services Requested.

3. Use of Data & Privacy

The data collected is used solely for the purpose of:

  • Processing your booking requests.

  • Communicating regarding relief shifts and professional services.

  • Internal record-keeping and invoicing.

We do not sell or lease hospital data to third parties. For more information on how we store data through our website provider, please see our Privacy Policy.

4. Accuracy of Information

You agree to provide accurate and current information. If you are submitting a request on behalf of a veterinary facility, you represent that you are at least 18 years of age and have the legal authority to share this information and request services on behalf of that entity. Our services are intended for professional veterinary entities and their authorized representatives.

5. Booking & Cancellation

All bookings made via this website are subject to the specific Booking and Cancellation Policies of Elevated Veterinary Solutions LLC. 6. Payment Methods

For services rendered following a booking, we accept payments via ACH transfer, professional check, or cash. Payment terms, including late fees, are governed by the specific Service Agreement signed prior to the shift. This website does not currently process direct payments. 

  • To cover the cost of processing a credit or charge card transaction, and pursuant to section 5-2-212, Colorado Revised Statutes, a seller or lessor may impose a processing surcharge in an amount not to exceed 2% of the total payment made for goods or services purchased or leased by use of a credit or charge card. A seller or lessor shall not impose a processing surcharge on payments made by use of cash, a check, or a debit card or redemption of a gift card.

7. Account Suspension or Cancellation

We reserve the right to permanently or temporarily terminate or suspend your access to our booking platform or intake forms without notice and liability for any reason, including if, in our sole determination, you violate any provision of these Terms or any applicable law or regulations.

8. Limitation of Liability

The information provided on this website is for general informational purposes. While we strive for 100% accuracy, Elevated Veterinary Solutions LLC is not liable for any errors or omissions in the website content or for any technical issues arising from the use of the Wix platform.

9. Intellectual Property

All content on this website, including logos, text, Service Agreements, and branded career timelines, is the property of Elevated Veterinary Solutions LLC and may not be used or reproduced without written consent.

10. Right to Modify Terms and Precedence of Service Agreement

We reserve the right to modify these website Terms and Conditions from time to time at our sole discretion. Your continued use of this website or our booking intake forms following any such change constitutes your acceptance of the revised Terms.

Important Note on Professional Services: These website Terms govern the use of this site only. All professional relief services are governed by a separate, formal Service Agreement. In the event of any conflict between these website Terms and a signed Service Agreement, the terms of the Service Agreement shall prevail and remain in full effect regarding termination, liability, and service changes.

11. Governing Law

These terms are governed by the laws of the State of Colorado, without regard to its conflict of law principles.
